How to submit a sample

If you are a patient, or potential participant: While we are very appreciative of your interest, please be aware, submitting a sample should be viewed as strictly a donation to medical research. It is not intended to benefit you personally nor will you receive any financial compensation for it. To insure that your rights are fully respected, we ask that you only submit samples after a complete discussion with your own physician or health care provider. This site is not intended to give you the level of informed consent that your own health provider can. That is why each and every sample must be submitted by a health care professional. We thank you for your understanding and cooperation.

For the health care professional: Thank you for your interest in submitting a participant's research sample. We will be happy to communicate with you directly to assist with the process. However, to ease your submission, all forms can be downloaded from this website. When ready to submit a sample, please:

  • Have the participant review and sign our consent form (download our consent form ). We are happy to provide additional information and/or clarification of our studies to assist you in providing informed consent.

  • Complete the appropriate patient data collection form.
    (AMDM - Brachydactyly - CACP - CCMS - ISH/JH - OPPG - PPD patients only).

  • Include contact information on how to reach you and the participant’s clinical diagnosis.

  • Sample requirements depend upon the participant’s disease and disease status.

    Collect:

  • Blood: 15-20 cc blood in lavender-top potassium EDTA tubes for adults stored and shipped at room temperature. 5-10 cc blood in lavender-top potassium EDTA tubes for children at room temperature.

  • For Synovial fluid: Expel synovial fluid (SF) from the syringe (without needle) into a polypropylene tube and immediately place on ice. Centrifuge at 1000 x g for 5 minutes at 4 C. and separate. The precipitate should then be topped with 10-20 ml of DMEM with antibiotics added. Since U.S. customs restricts the importation of samples containing non-human products, we do not recommend including FBS in the culture media. However, this can be added for shipments originating within the U.S. Ship immediately at room temperature. The supernatant should be transferred into a new tube and centrifuged again for 1 hour at 20,000 x g at 4 C. Decant supernatant and freeze at -20° C. Ship on dry ice. If fluid is grossly bloody, or has been frozen prior to centrifugation, please discard.

  • For Cartilage, Synovium, Skin, and Bone: Approximately half of the total tissue collected should be placed in DMEM (Dulbecco’s minimal essential media) with antibiotics. Since U.S. customs restricts the importation of samples containing non-human products, we do not recommend including FBS in the culture media. However, this can be added for shipments originating within the U.S. Ship immediately at room temperature. If this is going to arrive on the weekend, do not wait! Call ahead and we will make arrangements for special weekend receipt. Then take one quarter of the total tissue (or half of what is remaining) and flash-freeze in liquid nitrogen. Store at -80°C until you can ship on dry ice. Take the remaining one quarter of the total tissue and place in formalin. Ship at room temperature.

  • Cheek Swabs: request kit. It will be forwarded to the participant with instructions.
Samples and paper work can be sent by overnight delivery to:

There is no fee for submitting a sample, as this remains a research endeavor. Contacting us in advance of sending a sample, so we know when to expect it is always appreciated.

Please call with any questions, concerns, or comments (216) 368-5847 or email our Dr Warman. Additionally, if you know in advance that you will be taking a sample and would like us to provide the culture media, just let us know. ***
